Immunizations, Testing & Vaccines

California State University (CSU) requires all students to be current on a number of immunizations and to complete a risk assessment for tuberculosis (CSU Executive Order 803).

This executive order and its requirements apply to undergraduate and graduate students, students matriculated in self-support degree programs and K-12 students participating in concurrent enrollment courses entering the California State University.

You may submit your immunization records online via the Patient Portal. Please see instructions below.

  • Ensure your name and student ID are included on the forms before submission
  • Records must be in .pdf or .jpg format
  • Login to the Patient Portal with Saclink Credentials
  • Click on “Medical Clearances”
  • Click on "immunization Record Update" to submit records

If you are unable to submit your immunization record online you may fax to: (888) 318-5485.

Immunization Compliance Deadlines

Currently Enrolled Students: October 1, 2024

If required documentation is not received, a registration hold will be issued which will prevent registering for classes for the following semester

New or Transfer Students: 1st Day of Enrolled Semester

Required Documentation

Send one of the following documents to Student Health & Counseling Services with your name and student ID number clearly indicated on each document submitted:

  • Verification of immunity completed by healthcare provider
  • Photocopy of your childhood immunization record
  • Photocopy of your California High School transcript if immunization information is documented on transcript
  • A copy of a lab report showing proof of immunity by blood titer

Vaccines Offered

Human Papilloma Virus (HPV) is spread through intimate skin-to-skin contact and sexual activity. It’s highly transmissible and affects almost everyone at some point during their lifetime. Certain types of HPV can cause various cancers, including cervical, penile, anal, and oral cancers. HPV vaccines prevent 90% of HPV-related cancers. HPV vaccines are for all genders and can be administered up to age 45. Book a vaccine appointment with Student Health and Counseling Services through Patient Portal or call 916-278-6461. If you do not have insurance, you may be eligible for a patient assistance program to help cover the cost of the vaccine.

Travel Immunization

If you are planning to travel outside of the U.S., please review the required vaccinations for your destination at least 2 months prior to your departure. SHCS provides only vaccinations listed above.

MPX (Monkeypox) Vaccine: In response to this increase in cases, the U.S. Department of Health and Human Services and in the State of California (Governor Newsom) has declared MPX a public health emergency. In Sacramento County and surrounding areas, MPV vaccines are available in limited supply to those who meet risk eligibility criteria.
